Targeting the Skills Needed for
Reading and Handwriting
OT Evaluation
The first step into Summer Services is an Occupational Therapy Evaluation. An Occupational Therapist will interview the parent(s) by telephone or in person and then meet with the child for an in-person evaluation. Baseline data will be collected and/or standardized evaluations completed in order to clearly identify the child's current status in regard to reading and writing; more in-depth assessment of the child's visual-perceptual, cognitive (attention/memory/executive) and motor skills will be done as needed.

The goal is to be sure that the child is provided Summer Services that will help the child develop skills needed for success in the home and classroom. Children cannot attend the Group Sessions without having an OT Evaluation.

Evaluation- $150.00
Individual - $75.00/45 minutes
Group Sessions - $50.00/60 minute group
Group Sessions - $75.00/90 minute group

Payment arrangements will be made case by case, depending on insurance coverage. If insurance does not cover services, then the OT Evaluation may be paid for in person on the day of the evaluation or submitted through PayPal using the button below.
OT EVALUATION - ADDITIONAL INFORMATION
The Summer Services OT EVALUATION includes the parents' input as well as the clinical observations made by the Occupational Therapist. Any reports from the school may also provide valuable input to establish a starting point for services. The assessment can be done in our clinic or in your home. (We also have a structured assessment to identify your child's strengths and weaknesses that can be done at a park, if that is indicated!)
The Occupational Therapist may recommend a more detailed evaluation of skills that could include one or more of the following evaluation tools (NOT all of these are done on each child).

7. Motor-Free Visual Perception Test
8. Visual Screening
9. Binocular Vision Assessment
10. The Joy of Reading – Screening Assessment
11. The Roll Evaluation of Activities of Life (REAL)
