TRANSITIONAL SERVICES
Occupational Therapists provide information at a rate and time that a client can understand, respect the time and process older adults need to go through to make changes, and support "self-determination."
When an older adult understands all the positive factors a change can make, he or she is often more able to make a choice for change. Supporting decision-making by the older adult provides a sense of autonomy and dignity that can keep them strong....even in a new environment.
- Independent Living Center- Free Standing
- Independent Living in a Continuous Care Retirement Community (CCRC)
- Assisted Living- Home Based
- Assisted Living- Facility Based, typically in a CCRC
Or, if a client is considering moving, but is not ready for a major transition, OT can support
- Adding a home health aide or caregiver to provide the support needed to stay at home. OT can analyze the older adult's life needs, and make professional recommendations to help the person age "successfully."
- Recognizing the need for a professional organizer (not a therapist).
